This is an arsenic treatment monitoring agreement between Seacoast Analytical Services and the name below for the water treatment at the property address listed below.

This agreement includes:

Sample monitoring every 6 months, to begin 12 months after treatment installation. Date noted below.
Email notification will be sent at the beginning of each testing month to remind homeowners that kits are on the way.
Kits will be mailed directly to the homeowner with full sample instructions and return mailing included.
Reports will be emailed directly to the homeowner and water treatment company of choice if listed below.
Services may continue after treatment equipment is serviced but without the 12 month delay.
Extension of monitoring services can be purchased at the end of term.
In the case of the sale of this property, agreement does not transfer to a new property and no refund will be made.
New ownership of contracted property can continue with permission from the previous owner.
